Please be sure to keep our office informed of all address, phone number, and email address changes.  It is imperative that we have the most recent contact information so that there are not future difficulties in reaching anyone.  You may keep our office informed of these changes by referring to the Contact page in this website. 

 

 

 

Attention Requestors:

We have reviewed the Requests for Compensation. The Receiver has made the recommendations in the Request for Compensation Report that was filed with the court March 30, 2007. PLEASE SEE THE "NOTICE OF FILING RECEIVER'S COMPENSATION REPORT" TO VIEW THE RECOMMENDATIONS FOR YOUR REQUEST.

The Court hearing to give a ruling for the Requests for Compensation is to be on June 18, 2007. The Court will determine the final status of your request.

If your request has been Denied, we were unable to match the request information you submitted to a valid order in the Pharmalux/Size Max database. Possible reasons include:

Other Shipping Address (when product was sent)

Other Name used (i.e. middle name or nickname)

Other product purchased from a different company

You already received restitution from your credit card company or Pharmalux/Size Max by way of a credit

Your credit card was never charged (transaction declined)

If you feel the determination is an error and have documentation to support your amount requested, please be sure that information is in the Receiver’s office NO LATER THAN APRIL 16, 2007. Please send the information to:
